H1N1 Public Health Clinic Information

Clinic Date: November 4th

Place: Forest Hills School (Cafeteria)

Target Group: All School Aged Children, Parents of K-2 Children are encourage to attend with their child and are welcome to bring any younger siblings (ages 6months to 5 years of age) to receive vaccine as well. Only students who have returned a signed consent form will receive the vaccine.

Schedule: Public Health is planning of starting with the older children and working backwards (eg – beginning with grade 8 moving through backwards to Kindergarten). We have not received a finalized schedule yet.

Community Clinic: Simonds High School on November 6, 7, and 8 for any school aged children who missed their school clinic as well as all children between the ages of 6 months through 5 years. Parents who accompany their children to this clinic will be offer the vaccine at this time as well.
